What the inspection will not cover
Visual means visual. Pipe inside a wall, under a slab or below grade is not observed, and neither standard asks anyone to open a sealed access panel to reach it. InterNACHI names polybutylene piping in its exclusions outright.

What it is required to cover
The fixtures are a live test rather than a look. InterNACHI requires the inspector to run the water at every faucet, flush every toilet, and check that every sink, tub and shower drains; ASHI requires functional flow and functional drainage. If a fault shows up in ordinary use, this is the part of the inspection that finds it.

What to do about it
Where a report says a condition was observed but its extent could not be established, that is the sentence to act on. It means the boundary of the inspection was reached, not that the problem is small.
